Background Checks

In accordance with The Charter for the Protection of Children and Young
People revised and approved in June of 2005 by the United States
Conference of Catholic Bishops (USCCB) background checks must be
conducted for individuals volunteering with children or dependant adults
four or more times a year or when participating with these groups on
overnight activities. Background checks must also be conducted for all
employees working at diocesan entities. The Diocese of Davenport requires that forms, found in the links provided here, be completed and copies be kept on file at the parish as well as sent to the finance office for processing. The Acknowledgment and Consent form, copy of a driver’s license, Request for Dependent Adult Abuse Registry Information form, and Authorization for Release of Child Abuse Information form must be completed by required individuals in their entirety and sent in an envelope marked ‘Confidential’ to the finance office, Attn. Char Maaske. Background checks will be processed and entities billed for them at a later date. (A sample bill is below.) Periodically lists are sent to all parishes, schools, and other entities in the Diocese which detail all background checks that have been conducted for a particular entity. (A sample of these lists is below.) Lists must be reviewed and if an individual who meets the background check requirements is not on a list, the Diocese must be contacted to find out whether a background check has ever been processed for this person. If not, forms will need to be completed and submitted for the individual as soon as possible. Further instructions for the background check process can be found at the FAQ’s link also provided here.
